     _________ is a 10 year old adopted boy with a very difficult upbringing and extensive past history of physical abuse. He has several conditions occurring simultaneously, thus complicating his clinical presentation. He suffers from Attention Deficit Hyperactivity Disorder, combined type, Post Traumatic Stress Disorder, Reactive Attachment Disorder, and possible Oppositional Defiant Disorder.

     __________ has a tendency to get disorganized, and stress affects his symptoms. Currently, based on reports from school & his parents, he is more emotionally labile, disorganized, fearful & exhibiting more destructive behavior. he is presently in treatment at Child Outpatient Psychiatry Clinic in Stony Brook where he receives individual therapy, and medication management. __________s mother is also receiving parental counseling. We are continuing to work on medication adjustment, targeting the symptoms of ADHD, Anxiety & ODD.

     It appears that the stress of changing classes is a big contributor to the degree of academic difficulties ____________ is experiencing this school year. As we explore additional options to offer ____________ and his family the most intense and comprehensive treatment, we'd like for his school to strongly consider the following interventions:



1. An addition of 1:1 aide, to help him with his daily functioning and transition between classes.


2. A referral; to Sagamore 30 day program for intensive evaluation.
